This form should be used when no documentation can be tied to a specific decision you make in your company.  The old adadge of 'Get it in writting' holds true when litigation becomes a reality. 

Use this form to document the conversations that lead to decision regardless of what the topic is.  If possible, get everyone to sign, but at the very least, keep this as a record of your conversation.
